## Step 1 — Analyze the Operative Report
Extract the codeable elements from the operative narrative.
- Identify each distinct procedure performed — do not code the approach separately unless CPT defines it as a separate code.
- Determine the surgical approach: open, percutaneous, endoscopic, laparoscopic, robotic-assisted. Many CPT codes are approach-specific.
- Identify the anatomic site with laterality (left, right, bilateral) and specificity (which vertebral level, which digit, which artery).
- Note all specimens removed — size, margin status, and depth for excision codes.
- Document procedure duration, blood loss, and complications if they affect code selection.
- Identify any conversion from one approach to another (e.g., laparoscopic to open) — code the procedure actually completed.
- **Robotic-assisted procedures** --- Robotic-assisted surgery (da Vinci, Intuitive) is coded based on the surgical approach, not the technology: robotic-assisted laparoscopic cholecystectomy is coded as laparoscopic cholecystectomy (47562-47564). Do not separately report a code for the robotic technology unless a specific CPT code exists for the robotic approach

## Step 2 — Select Primary and Secondary CPT Codes
Assign codes from most resource-intensive to least.
- The primary procedure is the one with the highest RVU or most complexity — this is sequenced first on the claim and receives 100% payment.
- Additional procedures receive modifier 51 (multiple procedures) and are subject to the Multiple Procedure Payment Reduction (MPPR) — typically 50% of the fee schedule for the second procedure, 50% for subsequent.
- Add-on codes (designated with + in CPT) are never appended with modifier 51 and are not subject to MPPR.
- Verify parent code requirements for add-on codes — each add-on must be reported with its designated primary code.
- Check CPT instructional parenthetical notes beneath each code for "code also," "do not report together with," and "includes" language.
## Step 3 — Apply Surgical Modifiers
Select modifiers based on clinical circumstances.
- **Modifier 50 — Bilateral procedure**: Report once with modifier 50 (Medicare) or report the code twice with RT/LT modifiers (some commercial payers). Check payer preference.
- **Modifier 51 — Multiple procedures**: Applied to the second and subsequent procedures performed in the same session. Not used with add-on codes or modifier-51-exempt codes.
- **Modifier 59 / XE, XS, XP, XU — Distinct procedural service**: Used to bypass NCCI bundling edits when procedures are genuinely separate. Requires separate incision/excision, separate organ/structure, or separate encounter.
- XE: Separate encounter on the same date
- XS: Separate structure (different organ or anatomic site)
- XP: Separate practitioner
- XU: Unusual non-overlapping service not described by XE, XS, or XP
- **Modifier 62 — Two surgeons**: Each surgeon reports the same code with modifier 62. Each receives 62.5% of the allowable.
- **Modifier 66 — Surgical team**: Used for highly complex procedures requiring a team of physicians (e.g., organ transplants). Reported by each team member.
- **Modifier 80/81/82 — Surgical assistant**: 80 = assistant at surgery (full procedure), 81 = minimum assistant, 82 = assistant when qualified resident not available.
- **Modifier 78 — Return to OR during global period**: For complications requiring a return to the operating room for a related procedure. Resets the global period.
- **Modifier 79 — Unrelated procedure during global period**: For an unrelated procedure performed during the global period of a prior surgery. Initiates a new global period.
- **Laterality and specificity requirements** --- ICD-10-PCS and CPT both require anatomic specificity that must be verified against the operative report: which digit (finger/toe number), which vertebral level, which artery (named vessel), which side (left, right, bilateral), and which layer/depth. Generic coding ("finger" instead of "right ring finger") is a compliance risk and may trigger claim rejection
## Step 4 — Validate Against NCCI Edits
Check all code pairs and units for NCCI compliance.
- Run every combination of CPT codes on the same claim through the current NCCI PTP edit table.
- Column 1 code is the comprehensive code; Column 2 is the component code that is bundled.
- If a Column 2 code has a modifier indicator of "1," the code pair can be unbundled with an appropriate modifier (59, XE, XS, XP, XU) when documentation supports distinct services.
- If the modifier indicator is "0," the codes can NEVER be unbundled regardless of circumstances.
- Check Medically Unlikely Edits (MUEs) for unit limits — e.g., a unilateral procedure has MUE of 1 per day unless bilateral modifier is applied.
- Flag any code pair that requires unbundling and ensure operative report documentation explicitly supports the distinct service.
## Step 5 — Apply Global Surgical Period Rules
Manage the 0-day, 10-day, and 90-day global periods.
- **000 global**: No post-operative period. All subsequent care is separately billable.
- **010 global**: 10-day post-operative period. Includes visits on the day of surgery and 10 days following. No separate E/M billing during this period for related care.
- **090 global**: 90-day post-operative period. Includes 1 day pre-operative, day of surgery, and 90 days post-operative.
- E/M services during the global period for unrelated conditions require modifier 24 (unrelated E/M during global period) with a diagnosis code supporting the unrelated reason.
- Pre-operative E/M on the same day as a major surgery is included in the global package unless a separately identifiable service is documented with modifier 57 (decision for surgery).
- Modifier 58 applies to staged or planned procedures during the global period.
- **Inpatient vs. outpatient surgical coding** --- Coding conventions differ by setting: inpatient uses ICD-10-PCS for procedures (root operation-based), outpatient uses CPT/HCPCS. Some procedures are ASC-covered (per CMS ASC Covered Procedures List) and some are inpatient-only (per IPPS Inpatient Only List). Verify setting-appropriate code assignment

- Never code a procedure that is not documented in the operative report — "If it isn't documented, it wasn't done"
